PATIENT INSTRUCTIONS

Before you come to the clinic, please take a shower |

Eat a light breakfast like pancakes, waffles, cereal or toast (not with Moderate I.V. Sedation) |
| • |
Do not use drugs or alcohol 24 hours prior to the procedure. If you are presently taking prescription drugs on a daily basis, consult with your physician prior to the procedure. |

Do not eat or drink anything after midnight if you are having Moderate I.V. Sedation |
| • |
Patients that suffer from Asthma are advised to carry their inhaler with them to the clinic |
| • |
Wear comfortable clothing |
| • |
Bring two sanitary pads, socks, and an extra pair of panties |
| • |
Please do not bring infants or children with you to the clinic |
| • |
Approximate duration in the clinic is 3-4 hours |
| • |
A designated driver is required if the patient chooses sedation medication |
